Common use

It is also used off-label under specialist guidance for attention deficit hyperactivity disorder (ADHD), fatigue in multiple sclerosis, and adjunctively in depression with hypersomnia.

Dosage and direction

The recommended dose for narcolepsy or OSA is 200mg taken orally once daily in the morning. For SWSD, 200mg taken approximately one hour prior to the start of the work shift. Dosing may be adjusted within the range of 100mg to 200mg daily based on efficacy and tolerability. Tablets should be swallowed whole with water; administration with or without food does not significantly affect absorption but may reduce mild gastrointestinal discomfort.

Precautions

Patients should be advised that Modaheal may impair the ability to engage in potentially hazardous activities. Use with caution in those with a history of psychosis, depression, mania, or cardiovascular disease. Hepatic impairment necessitates dose reduction. Not recommended in severe renal impairment. Pregnancy Category C: use only if potential benefit justifies potential risk. Excreted in breast milk—discontinue nursing or discontinue drug.

Contraindications

Hypersensitivity to modafinil or armodafinil or any component of the formulation. Contraindicated in patients with history of left ventricular hypertrophy or ischemic ECG changes, chest pain, or arrhythmia. Should not be used concomitantly with monoamine oxidase (MAO) inhibitors due to risk of hypertensive crisis.

Possible side effects

Rare but serious: Stevens-Johnson syndrome, angioedema, multiorgan hypersensitivity, psychiatric symptoms (e.g., aggression, hallucinations). Patients should report any severe skin reaction or mood changes immediately.

Drug interaction

Modaheal is a moderate inducer of CYP3A4 and inhibitor of CYP2C19. May reduce efficacy of oral contraceptives, cyclosporine, midazolam, triazolam, and certain anticoagulants. Concentrations of modafinil may be increased by CYP3A4 inhibitors (e.g., ketoconazole) and decreased by inducers (e.g., carbamazepine). May interact with serotonergic drugs.

Missed dose

If a dose is missed, it should be taken as soon as possible unless it is close to the time of the next scheduled dose. Doubling the dose is not recommended. For shift work disorder, if missed prior to shift, take as soon as remembered but not within 4 hours of sleep time.

Overdose

Symptoms may include insomnia, agitation, confusion, tachycardia, hypertension, and nausea. There is no specific antidote. Management is supportive, including ECG monitoring and symptomatic treatment. Hemodialysis is not expected to enhance elimination.

Storage
