Common use
Phexin is commonly prescribed for bacterial infections including pharyngitis, tonsillitis, otitis media, bronchitis, pneumonia, skin and soft tissue infections (e.g., cellulitis, impetigo), urinary tract infections (cystitis, pyelonephritis), and bone infections. It is also used prophylactically before certain surgical procedures to prevent postoperative infections.
Dosage and direction
Dosage must be individualized based on infection type, severity, patient age, and renal function.
Adults: Typical dose is 250–500 mg every 6 hours, or 500 mg–1 g every 12 hours.
Children: 25–50 mg/kg/day divided into 2–4 doses. Maximum daily dose should not exceed 4 g.
Oral suspension should be shaken well before use. Tablets and capsules may be taken with food to minimize gastrointestinal upset. Complete the full course even if symptoms improve earlier.
Precautions
- Use with caution in patients with renal impairment; dosage adjustment may be necessary.
- Prolonged use may result in fungal or bacterial superinfection.
- May cause false-positive urine glucose tests; use glucose-specific testing methods.
- Not recommended during pregnancy unless clearly needed; consult healthcare provider.
- Breastfeeding mothers should use under medical supervision—cefalexin is excreted in milk.
Contraindications
Phexin is contraindicated in patients with known hypersensitivity to cefalexin, cephalosporins, penicillins, or other beta-lactam antibiotics. Do not use in cases of previous severe allergic reaction (e.g., anaphylaxis) to any antibiotic.
Possible side effect
Common side effects may include diarrhea, nausea, vomiting, abdominal pain, indigestion, and dizziness. Less frequently, rash, urticaria, genital itching, vaginitis, transient hepatitis, or eosinophilia may occur. Rare but serious side effects include pseudomembranous colitis, Stevens-Johnson syndrome, and anaphylaxis.
Drug interaction
- Probenecid may reduce renal excretion of cefalexin, increasing its blood levels.
- Concurrent use with aminoglycosides or loop diuretics may increase nephrotoxicity risk.
- May potentiate the effects of oral anticoagulants (e.g., warfarin).
- Antacids containing aluminum or magnesium may reduce absorption.
Missed dose
If a dose is missed, take it as soon as remembered. If it is almost time for the next dose, skip the missed dose and resume the regular schedule. Do not double the dose.
Overdose
Symptoms of overdose may include nausea, vomiting, epigastric distress, diarrhea, hematuria, or seizures. Management is supportive; gastric lavage and symptomatic treatment may be indicated. Hemodialysis can aid in removal of the drug.
Storage
Store at room temperature (15–30°C), away from light, moisture, and heat. Keep oral suspension refrigerated after reconstitution and discard unused portion after 14 days. Keep out of reach of children.
