Propecia, with the active ingredient finasteride, is a prescription medication specifically developed to treat male pattern hair loss (androgenetic alopecia). It represents a cornerstone of dermatological and trichological practice for addressing this common condition. By targeting the underlying hormonal cause of hair loss, Propecia offers a scientifically validated approach to slowing progression, promoting hair regrowth, and improving hair count in a significant proportion of men. Its efficacy and safety profile are supported by extensive clinical trials and decades of real-world use.

Features

  • Active Ingredient: Finasteride 1mg.
  • Drug Class: 5-alpha-reductase inhibitor (Type II).
  • Administration: Oral tablet.
  • Prescription Status: Rx-only medication.
  • Mechanism of Action: Specifically inhibits the conversion of testosterone to dihydrotestosterone (DHT) in the scalp and prostate.

Benefits

  • Halts the Progression of Hair Loss: By significantly reducing scalp DHT levels, the primary hormonal driver of follicular miniaturization in genetically susceptible men.
  • Promotes Hair Regrowth: Clinical studies demonstrate an increase in hair count and visible improvement in hair growth for the vertex (top of head) and mid-scalp areas.
  • Prevents Further Follicular Miniaturization: Protects hair follicles from the damaging effects of DHT, helping to maintain existing hair quality and diameter.
  • Long-Term Efficacy: Sustained treatment is associated with maintained or improved hair count over many years, as shown in five-year clinical data.
  • Non-Surgical Intervention: Provides a systemic, pharmacological option for men seeking to avoid or complement surgical hair restoration procedures.
  • Well-Tolerated by Most Men: The majority of users experience no significant side effects, allowing for continuous, effective long-term management of hair loss.

Common use

Propecia is indicated for the treatment of male pattern hair loss in men only. It is used to treat hair loss on the vertex (top of the head) and the anterior mid-scalp area. It is not indicated for use in women or children. Treatment is most effective for men who begin therapy early in the hair loss process, as it is more effective at retaining existing hair and stimulating regrowth in areas with thinning hair rather than completely bald, smooth scalp.

Dosage and direction

The recommended dosage is one 1mg tablet administered orally once daily, with or without food. For optimal results, daily use is required. Clinical studies indicate that a minimum of three months of daily dosing is necessary to observe any effect, and at least 6-12 months of continuous therapy are typically required to assess full response. Treatment must be continued to sustain benefit. If treatment is stopped, any gained hair will typically be lost within 12 months.

Precautions

  • Propecia is for use by adult men only. Women who are or may potentially be pregnant must not handle crushed or broken tablets due to the risk of absorption and potential teratogenic effects (abnormalities of the external genitalia) on a male fetus.
  • A digital rectal examination (DRE) and/or assessment for prostate cancer may be appropriate before initiating and periodically during treatment, as finasteride can lower PSA levels.
  • Patients should be advised that they may need to use Propecia for several months to see a benefit.
  • Patients with liver function impairment should be used with caution, as finasteride is metabolized extensively in the liver.

Contraindications

  • Hypersensitivity to finasteride or any other component of Propecia.
  • Use in women, particularly those who are pregnant or of childbearing potential.
  • Use in pediatric patients.

Possible side effect

The following adverse reactions are based on clinical trials and post-marketing experience. The most common side effects affect a small percentage of users and are often reversible upon discontinuation of the drug.

  • Sexual Dysfunction: Decreased libido (1.8%), erectile dysfunction (1.3%), and decreased volume of ejaculate (1.2%). These events typically resolve in men who discontinue treatment and in many who continue therapy.
  • Reproductive System: Breast tenderness and enlargement (gynecomastia).
  • Hypersensitivity Reactions: Rash, itching, hives, and swelling of the lips and face.
  • Testicular Pain.
  • Depression. A causal relationship has not been firmly established.
  • Persistent sexual side effects, such as erectile dysfunction, low libido, and problems with orgasm, have been reported post-marketing after discontinuation of treatment. The frequency and mechanism of these persistent side effects are not well understood.

Drug interaction

Formal drug interaction studies have not revealed any clinically significant interactions. However, caution is advised with:

  • Theoretical interactions with other drugs that are extensively metabolized by the CYP3A4 enzyme system.
  • No dosage adjustment is recommended when used concomitantly with terazosin, warfarin, digoxin, theophylline, or propranolol, based on specific interaction studies.
  • 7-alpha-(4-chlorobenzyl)-7β-hydroxy-3-oxo-4-androstene-17β-carboxylic acid is a known metabolite, but its clinical significance in interactions is minimal.

Missed dose

If a dose is missed, it should be skipped. The patient should not take a double dose to make up for the missed one. Simply resume the usual schedule of one tablet once daily the next day.

Overdose

In clinical trials, doses of finasteride up to 400 mg daily for three months have been administered without observed adverse effects. Specific symptoms of overdose are not well-established. In the event of a suspected overdose, supportive care and general symptomatic treatment are indicated. As finasteride is highly protein-bound, it is not expected to be dialyzable.

Storage

Store Propecia tablets at room temperature, between 15°C and 30°C (59°F and 86°F). Keep the bottle tightly closed and stored in a dry place. Keep out of reach of children and pets.

Reviews

  • “As a dermatologist with over twenty years of experience treating androgenetic alopecia, Propecia remains a first-line therapeutic option. The data from long-term studies is compelling, and for the vast majority of my male patients, it effectively stabilizes their hair loss with minimal issues. Patient education on the onset of action and potential side effects is paramount for adherence and satisfaction.” – Dr. Evan R., MD, Dermatology
  • “I’ve been on Propecia for eight years. It hasn’t given me a full head of hair from my teenage years, but it has completely stopped my progressive thinning. My hairline is stable, and the crown is much fuller. I experienced some initial slight libido changes that subsided after the first few months. For me, the trade-off is unquestionably worth it.” – Mark T., Patient
  • “The biochemical precision of finasteride’s mechanism is what makes it such a valuable tool. It doesn’t just treat a symptom; it directly inhibits the key hormone responsible for the pathology of male pattern baldness. This targeted approach is why it has stood the test of time in our pharmacological arsenal.” – Dr. Sarah L., PhD, Clinical Pharmacologist
