Tiova Rotacap is a high-efficacy, long-acting muscarinic antagonist (LAMA) designed for the maintenance treatment of chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema. Each rotacap contains Tiotropium Bromide, which provides sustained bronchodilation by targeting airway smooth muscle, reducing bronchospasm, and improving lung function over 24 hours. It is delivered via the Rotahaler device for deep lung deposition, ensuring optimal therapeutic effect with consistent dosing. Clinically validated, Tiova Rotacap helps patients achieve better symptom control, enhanced exercise tolerance, and reduced exacerbation frequency, supporting long-term respiratory health.

Common use
Tiova Rotacap is indicated for the long-term, once-daily maintenance treatment of bronchospasm associated with chronic obstructive pulmonary disease (COPD). It is used to reduce exacerbations and improve breathing in patients diagnosed with chronic bronchitis, emphysema, or overlapping conditions. It is not intended for immediate relief of acute bronchospasm.
Dosage and direction
The recommended dosage is one Tiova Rotacap (18 mcg) inhaled once daily, at the same time each day, using the Rotahaler device. Do not swallow the capsule. Instructions for use:
- Open the Rotahaler and insert the capsule into the chamber.
- Close the device and pierce the capsule by pressing the side buttons firmly.
- Breathe out fully, place the mouthpiece between lips, and inhale deeply and steadily.
- Hold breath for up to 10 seconds, then exhale slowly.
- Repeat inhalation if any powder remains. Discard empty capsule.
Do not exceed the prescribed dose.
Precautions
- Not for use in the treatment of acute asthma or bronchospasm.
- Use with caution in patients with narrow-angle glaucoma, prostatic hyperplasia, or bladder-neck obstruction.
- Monitor for signs of paradoxical bronchospasm; discontinue if it occurs.
- Avoid contact with eyes; may cause pupil dilation and blurred vision.
- Caution advised in patients with severe renal impairment (creatinine clearance ≤50 mL/min).
- Inform healthcare provider of all current medications before starting therapy.
Contraindications
Tiova Rotacap is contraindicated in patients with:
- Hypersensitivity to tiotropium bromide, atropine, or its derivatives.
- History of severe allergic reactions to milk proteins (contains lactose).
- Acute episodes of bronchospasm where rapid short-acting bronchodilation is required.
Possible side effects
Common side effects may include:
- Dry mouth
- Constipation
- Upper respiratory tract infection
- Pharyngitis
- Sinusitis
Less common but serious side effects:
- Glaucoma (eye pain, blurred vision)
- Urinary retention
- Tachycardia
- Allergic reactions (rash, itching, swelling)
- Paradoxical bronchospasm
Drug interaction
- Concurrent use with other anticholinergic agents may potentiate side effects.
- May interact with drugs metabolized by CYP2D6 (e.g., paroxetine, flecainide).
- Use caution with beta-blockers, which may reduce bronchodilator efficacy.
- No significant interaction observed with methylxanthines or oral corticosteroids.
Missed dose
If a dose is missed, take it as soon as remembered the same day. If it is nearly time for the next dose, skip the missed dose and resume the regular schedule. Do not double the dose.
Overdose
Overdose may lead to intensified anticholinergic effects such as dry mouth, visual disturbances, tachycardia, and urinary retention. Treatment is supportive; there is no specific antidote. In case of suspected overdose, seek medical attention immediately.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep the capsules in the original blister pack until use. Do not store in bathrooms or damp areas. Keep out of reach of children and pets.
