Common use

Assurans is indicated for the treatment of erectile dysfunction (ED) in adult men. Contraindications

Assurans is contraindicated in patients with:

  • A known hypersensitivity to Sildenafil Citrate or any of the tablet’s excipients.
  • The concurrent use of organic nitrates or nitric oxide donors (e.g., amyl nitrite) in any form, either regularly or intermittently, due to the risk of a precipitous and potentially fatal drop in blood pressure.
  • The concurrent use of guanylate cyclase stimulators, such as riociguat.

Possible side effect

  • Rare (≥1/10,000 to <1/1,000): Priapism (a prolonged and painful erection lasting more than 4 hours; requires immediate medical attention to prevent permanent tissue damage), hypersensitivity reactions (including skin rashes), sudden hearing loss, myocardial infarction, stroke. Patients should be advised to discontinue use and consult their physician if any side effect becomes severe or bothersome.

Drug interaction

Assurans can potentiate the hypotensive effects of nitrates and antihypertensive medications. Concomitant use is contraindicated with nitrates. Caution is advised when co-administered with alpha-blockers (e.g., doxazosin, tamsulosin) as this may lead to symptomatic postural hypotension. Assurans is primarily metabolized by the cytochrome P450 (CYP) isoforms 3A4 (major route) and 2C9 (minor route). Consequently, inhibitors of CYP3A4 (e.g., ketoconazole, itraconazole, ritonavir, erythromycin, grapefruit juice) may increase Sildenafil plasma concentrations, while inducers of CYP3A4 (e.g., rifampicin, carbamazepine, phenytoin, phenobarbital, St. John’s Wort) may decrease Sildenafil plasma concentrations.

Missed dose

Assurans is not intended for chronic daily dosing. It is taken on an as-needed basis. Therefore, the concept of a “missed dose” does not apply. Do not take an extra dose to make up for a missed opportunity. Adhere to the maximum dosing frequency of once per day.

Overdose

In cases of overdose, the expected clinical presentation would be an extension of the drug’s known adverse effects, including severe hypotension, syncope, and prolonged erection. Standard supportive measures should be employed as required. Given that Sildenafil is highly protein-bound, it is not expected to be dialyzable. Management of priapism should be immediate and may require corporal irrigation and, in severe cases, surgical intervention to prevent irreversible damage. In cases of severe hypotension, cardiovascular support with fluid replacement and, if necessary, vasopressor agents (e.g., phenylephrine, epinephrine) may be considered.

Storage

Store Assurans tablets below 30°C (86°F). Keep the medication in its original blister pack to protect it from light and moisture. Keep out of the reach and sight of children. Do not use Assurans after the expiry date which is stated on the packaging after “EXP.” The expiry date refers to the last day of that month. Do not dispose of medications via wastewater or household waste. Ask your pharmacist how to dispose of medicines no longer required.
