Detrol: Effective Relief from Overactive Bladder Symptoms
Detrol (tolterodine tartrate) is a prescription medication specifically formulated to manage the symptoms of overactive bladder (OAB). As an antimuscarinic agent, it works by relaxing the bladder muscle, which helps to reduce episodes of urgency, frequency, and urge incontinence. This medication is a cornerstone in the pharmacological management of OAB, offering a balance of efficacy and tolerability for appropriate patients. Clinical studies and extensive post-marketing surveillance support its role in significantly improving quality of life for those affected by this disruptive condition.
Features
- Active ingredient: Tolterodine tartrate
- Available in immediate-release (Detrol) and extended-release (Detrol LA) formulations
- Standard immediate-release dosage: 1 mg and 2 mg tablets
- Standard extended-release dosage: 2 mg and 4 mg capsules
- Prescription-only medication
- Muscarinic receptor antagonist mechanism of action
Benefits
- Reduces Urinary Urgency: Effectively diminishes the sudden, strong need to urinate immediately.
- Decreases Urination Frequency: Helps increase the time between bathroom visits, allowing for longer periods of uninterrupted activity and sleep.
- Manages Urge Incontinence: Significantly reduces or eliminates episodes of involuntary urine leakage associated with a strong urge to void.
- Improves Quality of Life: By controlling core OAB symptoms, it enables greater social, occupational, and personal freedom.
- Convenient Dosing: The extended-release formulation allows for once-daily administration, improving adherence.
Common use
Detrol is primarily indicated for the treatment of overactive bladder (OAB) with symptoms of urge urinary incontinence, urgency, and urinary frequency. It is prescribed for adults who experience these symptoms due to idiopathic or neurogenic causes. The goal of therapy is to restore a more normal bladder function pattern, thereby reducing the psychosocial and physical burden associated with the condition. Treatment is typically initiated after behavioral modifications (e.g., bladder training, fluid management) have provided insufficient relief.
Dosage and direction
The dosage of Detrol must be individualized based on patient response and tolerability.
- Detrol (immediate-release): The recommended initial dose is 2 mg twice daily. The dose may be lowered to 1 mg twice daily based on individual response and tolerability, particularly in patients with significantly reduced liver function or who are taking certain CYP3A4 inhibitors.
- Detrol LA (extended-release): The recommended dose is 4 mg taken once daily with liquids and swallowed whole. The dose may be lowered to 2 mg once daily based on individual response and tolerability.
- Administration with food is optional, as it does not significantly affect the absorption of the medication.
- It is crucial to follow the prescribing physician’s instructions exactly. Do not crush, chew, or split the extended-release capsules.
Precautions
Before starting Detrol, patients should discuss their full medical history with their healthcare provider. Special caution is advised for individuals with:
- Liver or Kidney Impairment: Dosage adjustments are necessary for patients with significantly reduced hepatic function or severe renal impairment.
- Bladder Outlet Obstruction: Conditions like benign prostatic hyperplasia (BPH) may increase the risk of urinary retention.
- Gastrointestinal Disorders: Patients with gastrointestinal obstructive disorders (e.g., pyloric stenosis) should use Detrol with caution due to the risk of decreased gastrointestinal motility.
- Myasthenia Gravis: Tolterodine can exacerbate the symptoms of this neuromuscular condition.
- Controlled Narrow-Angle Glaucoma: Use with caution and under close supervision by an ophthalmologist.
- Pregnancy and Breastfeeding: Detrol should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is not known if tolterodine is excreted in human milk; a decision should be made to discontinue nursing or discontinue the drug.
Contraindications
Detrol is contraindicated in patients with:
- Urinary Retention: Patients who are unable to empty their bladder.
- Gastric Retention: Conditions that significantly slow or stop the movement of food through the stomach and intestines.
- Uncontrolled Narrow-Angle Glaucoma: A severe form of glaucoma where pressure inside the eye rises very quickly.
- Known Hypersensitivity: A history of allergic reaction to tolterodine tartrate or any component of the formulation.
Possible side effect
Like all medications, Detrol can cause side effects, although not everybody gets them. The most common side effects are related to its anticholinergic properties and are usually mild to moderate.
- Very Common (β₯1/10): Dry mouth, headache.
- Common (β₯1/100 to <1/10): Dry eyes, dyspepsia (indigestion), constipation, abdominal pain, dizziness, somnolence (drowsiness).
- Uncommon (β₯1/1,000 to <1/100): Nervousness, paraesthesia (tingling sensation), fatigue, tachycardia (fast heart rate), palpitations, difficulty emptying the bladder, blurred vision, gastroesophageal reflux disease (GERD).
- Rare (<1/1,000): Allergic reactions including rash, itching, and urticaria (hives); confusion; hallucinations; angioedema.
Patients should contact their doctor if any side effects become severe or do not go away.
Drug interaction
Detrol is primarily metabolized by the cytochrome P450 enzyme system, specifically CYP2D6 and CYP3A4. Concomitant use with other drugs that interact with these enzymes can alter its plasma concentration.
- Potent CYP3A4 Inhibitors: Drugs like ketoconazole, itraconazole, clarithromycin, and ritonavir can significantly increase tolterodine plasma levels. Co-administration is not recommended.
- Other Anticholinergic Drugs: Concurrent use with other medications that have anticholinergic effects (e.g., oxybutynin, some antidepressants, antihistamines) may increase the frequency and/or severity of side effects like dry mouth, constipation, and blurred vision.
- Drugs Metabolized by CYP2D6: Tolterodine may inhibit the metabolism of other drugs that are substrates for CYP2D6, such as flecainide, thioridazine, and some antidepressants. Dose adjustment of these concomitant medications may be required.
Always provide your doctor and pharmacist with a complete list of all prescription, over-the-counter, and herbal products you are taking.
Missed dose
- If a dose of Detrol (immediate-release) or Detrol LA (extended-release) is missed, it should be taken as soon as it is remembered.
- However, if it is almost time for the next scheduled dose, skip the missed dose and take the next dose at the regular time.
- Do not take a double dose to make up for a missed one, as this increases the risk of side effects.
Overdose
Overdose with tolterodine can potentially lead to severe central anticholinergic effects.
- Symptoms: May include severe dry mouth, blurred vision, tachycardia, dizziness, difficulty breathing, severe drowsiness, agitation, nervousness, hallucinations, and seizures.
- Management: In the event of a suspected overdose, seek immediate medical attention or contact a Poison Control Center. Treatment is symptomatic and supportive. ECG monitoring is recommended. Activated charcoal may be administered if presented early.
Storage
- Store Detrol tablets and Detrol LA capsules at room temperature (20Β°C to 25Β°C or 68Β°F to 77Β°F), in a dry place, protected from light and moisture.
- Keep all medications out of the reach of children and pets.
- Do not flush medications down the toilet or pour them into a drain unless instructed to do so. Properly discard this product when it is expired or no longer needed through a medicine take-back program.

Reviews
“After struggling with frequent and urgent trips to the bathroom for years, my urologist prescribed Detrol LA. The change has been remarkable. I can now sit through an entire movie or a long meeting without anxiety. The dry mouth was noticeable at first but has lessened over time. It’s given me my confidence back.” β Sarah T., 58
“As a physician, I prescribe Detrol frequently for appropriate patients with OAB. The extended-release formulation offers excellent 24-hour coverage with a favorable side effect profile for most. It’s a reliable first-line option in our therapeutic arsenal, and patient satisfaction is generally high when expectations regarding potential side effects are managed from the outset.” β Dr. Evan R., Urologist
“I was hesitant to start medication, but the constant urgency was impacting my job. Detrol has significantly reduced my symptoms. I did experience some initial constipation, but increasing my water and fiber intake helped manage it. The benefits far outweigh this minor side effect for me.” β Mark D., 45
