Common use

Imitrex is indicated for the acute treatment of migraine attacks with or without aura in adults. It is not intended for the prophylactic management of migraine or for the treatment of cluster headaches, although other triptans may be used for that indication. Patients typically use Imitrex at the onset of migraine symptoms for optimal results, though it remains effective when used later in an attack. It should not be used in patients with certain cardiovascular risk factors or uncontrolled hypertension.

Dosage and direction

Tablets: The recommended initial dose is 25mg, 50mg, or 100mg taken with fluids as a single dose. If headache returns, a second dose may be taken after 2 hours, not to exceed 200mg in 24 hours.

Injection: A single 6mg subcutaneous injection is recommended, which may be repeated after 1 hour if needed (maximum 12mg/24 hours).

Nasal Spray: A single 5mg, 10mg, or 20mg dose sprayed into one nostril, which may be repeated after 2 hours if needed (maximum 40mg/24 hours).

The minimum effective dose should be established for each patient. Administration in a reclined position is advised for the injection to minimize dizziness.

Precautions

Patients should be thoroughly evaluated for cardiovascular risk factors before prescribing Imitrex. Use with caution in patients with controlled hypertension, impaired hepatic function, or risk factors for coronary artery disease. May cause dizziness or drowsiness—patients should not drive or operate machinery until they know how the medication affects them. Not recommended for use in elderly patients or those with significant renal impairment without careful consideration. Use during pregnancy only if clearly needed; caution advised during breastfeeding.

Contraindications

Imitrex is contraindicated in patients with: ischemic heart disease, history of myocardial infarction, or coronary artery vasospasm; uncontrolled hypertension; cerebrovascular syndromes including strokes and TIAs; peripheral vascular disease; severe hepatic impairment; hypersensitivity to sumatriptan or any component of the formulation. Drug interaction

Significant interactions may occur with: MAO inhibitors (contraindicated within 2 weeks of use), other triptans or ergot derivatives (24-hour separation required), SSRIs/SNRIs (increased risk of serotonin syndrome), and medications that affect serotonin metabolism. Use with caution with other vasoconstrictors. The injection formulation may interact with drugs affecting cardiac rhythm.

Missed dose

Imitrex is taken on an as-needed basis for acute migraine attacks and does not follow a regular dosing schedule. There is no need to make up a missed dose. The medication should only be taken when experiencing a migraine attack, following the prescribed dosing instructions for that episode.

Overdose

Symptoms of overdose may include hypertension, syncope, reflex bradycardia, seizures, and cardiovascular events. In cases of suspected overdose, provide supportive care including continuous ECG monitoring for at least 20 hours and symptomatic treatment. Hemodialysis is unlikely to be effective due to high protein binding. Contact poison control center for latest management recommendations.

Storage

Store at controlled room temperature (20-25°C or 68-77°F). Protect from light and moisture. Keep injection and nasal spray in original packaging until use. Do not freeze.
