Common use

Nasonex Nasal Spray is primarily indicated for the treatment of nasal symptoms of seasonal allergic rhinitis and perennial allergic rhinitis in adults and children ages 2 years and older. It is also approved for the prophylaxis of seasonal allergic rhinitis symptoms when initiated 2-4 weeks prior to anticipated onset of the pollen season. Additionally, it may be prescribed for the treatment of nasal polyps in patients 18 years of age and older.

Dosage and direction

For allergic rhinitis in adults and children 12 years and older: 2 sprays in each nostril once daily (total dose 200 mcg). When symptoms are controlled, dosage may be reduced to 1 spray in each nostril once daily. For children 2-11 years: 1 spray in each nostril once daily (total dose 100 mcg). For seasonal allergic rhinitis prophylaxis: begin 2-4 weeks prior to anticipated pollen season. Shake well before use. Prime the pump before first use by pressing 10 times until a fine spray appears. If not used for 14 days or longer, reprime with 2 sprays.

Precautions

Use with caution in patients with recent nasal septum ulcers, nasal surgery, or nasal trauma until healing has occurred. Monitor patients for signs of adrenal suppression when using higher than recommended doses or with concomitant systemic corticosteroids. Nasal corticosteroids may slow growth velocity in pediatric patients; monitor growth routinely. Use during pregnancy only if potential benefit justifies potential risk. Exercise caution in patients with active or quiescent tuberculosis infections, untreated fungal, bacterial, or viral systemic infections, or ocular herpes simplex.

Contraindications

Hypersensitivity to mometasone furoate or any component of the formulation. Not recommended for the treatment of non-allergic rhinitis. Contraindicated in patients with untreated localized nasal infections. Should not be used for emergency treatment of acute asthma episodes or other acute respiratory conditions.

Possible side effects

Common side effects (≥1%) include: headache, viral infection, pharyngitis, epistaxis (blood-tinged mucus or frank bleeding), coughing, and upper respiratory tract infection. Less common side effects include: nasal burning/irritation, sinusitis, nausea, dysmenorrhea, and musculoskeletal pain. Rare but serious side effects may include: nasal septum perforation, ocular hypertension/glaucoma, cataracts, impaired wound healing, and signs of systemic corticosteroid effects with prolonged use.

Drug interaction

Formal drug interaction studies have not been conducted with Nasonex Nasal Spray. However, caution is advised when using with other corticosteroids due to potential additive systemic effects. Ketoconazole and other potent CYP3A4 inhibitors may increase mometasone plasma concentrations. No clinically significant interactions have been observed with commonly prescribed medications including antihistamines, decongestants, or antibiotics.

Missed dose

If a dose is missed, use it as soon as remembered. However, if it is almost time for the next dose, skip the missed dose and continue with the regular dosing schedule. Do not double the dose to make up for a missed one. Consistent daily use provides optimal therapeutic效果.

Overdose

Acute overdose with Nasonex Nasal Spray is unlikely due to limited systemic absorption. Single doses up to 20 times the recommended dose have been administered without adverse effects. Chronic overdose may lead to systemic corticosteroid effects including hypercorticism and adrenal suppression. In case of suspected overdose, discontinue treatment and provide supportive care. There is no specific antidote.

Storage

Store at room temperature between 15-30°C (59-86°F). Do not freeze. Keep the bottle upright and protected from light. Discard after 120 actuations following initial priming, as the amount of mometasone furoate per actuation may thereafter be substantially less than 50 mcg. Keep out of reach of children.
