Common use
Nexium is commonly prescribed for the short-term treatment (4 to 8 weeks) of erosive esophagitis, as well as for the maintenance of healing and symptom control in GERD. It is also used in combination with antibiotics for the eradication of Helicobacter pylori to reduce the risk of duodenal ulcer recurrence. Additionally, it is indicated for the risk reduction of NSAID-associated gastric ulcers in patients requiring continuous NSAID therapy. Over-the-counter Nexium is used for the self-treatment of frequent heartburn (occurring two or more days per week).
Dosage and direction
The recommended adult dosage for healing erosive esophagitis is 20 mg or 40 mg once daily for 4 to 8 weeks. For maintenance of healing of erosive esophagitis and symptomatic GERD, the dose is 20 mg once daily. For H. pylori eradication, Nexium 40 mg once daily is used in combination with amoxicillin and clarithromycin for 10 days. To reduce the risk of NSAID-associated gastric ulcers, the dose is 20 mg or 40 mg once daily for up to 6 months. Nexium should be taken at least one hour before a meal, preferably in the morning. The capsules should be swallowed whole; they must not be crushed, chewed, or split.
Precautions
Patients should inform their healthcare provider if they have liver disease, as dosage adjustments may be necessary. Long-term use (especially beyond one year) may lead to decreased magnesium levels; regular monitoring is advised. There is an increased risk of bone fractures with prolonged high-dose therapy. Nexium may mask symptoms of gastric malignancy; appropriate diagnostic evaluation should be conducted if unexpected weight loss, recurrent vomiting, or dysphagia occurs. Contraindications

It can increase systemic concentrations of drugs metabolized by CYP2C19, such as diazepam, citalopram, and phenytoin. Concomitant use with clopidogrel may reduce the antiplatelet activity of clopidogrel. Caution is advised with methotrexate due to increased methotrexate levels. Missed dose
If a dose is missed, it should be taken as soon as remembered, unless it is nearly time for the next scheduled dose. In that case, the missed dose should be skipped, and the regular dosing schedule resumed. Doubling the dose to make up for a missed one is not recommended.
Overdose
There is limited experience with human overdose. Potential manifestations may reflect the known effects of esomeprazole, such as drowsiness, blurred vision, tachycardia, nausea, sweating, flushing, headache, and dry mouth. Specific antidotes are not known; treatment should be symptomatic and supportive. Hemodialysis is not likely to be effective due to high protein binding.
Storage
Store at room temperature (20°C to 25°C or 68°F to 77°F), with excursions permitted between 15°C and 30°C (59°F to 86°F). Keep the container tightly closed and protect from moisture and light. Keep out of reach of children and pets. Do not use beyond the expiration date printed on the packaging.
