Common use
Ventolin HFA is primarily indicated for the treatment or prevention of bronchospasm in patients aged 4 years and older with reversible obstructive airway disease, such as asthma. It is also used for the prevention of exercise-induced bronchospasm (EIB). It is classified as a rescue inhaler and is intended for immediate symptom relief. It is not meant for daily maintenance control of asthma; its use for treating acute symptoms more than twice a week may indicate poor asthma control and should prompt a re-evaluation of the patient’s maintenance therapy regimen.
Dosage and direction
The dosage of Ventolin HFA should be individualized based on patient need and response. Shake the inhaler well before each use. Prime the inhaler before first use or if it has not been used for more than 2 weeks by releasing 4 test sprays into the air away from the face.
For treatment of acute bronchospasm: • Adults and children 4 years of age and older: The usual dose is 2 inhalations every 4 to 6 hours as needed. • In some cases, 1 inhalation every 4 hours may be sufficient.
For prevention of exercise-induced bronchospasm: • Adults and children 4 years of age and older: 2 inhalations 15 to 30 minutes before exercise.
Administration Technique:
- Breathe out fully, expelling as much air from your lungs as possible.
- Place the mouthpiece between your teeth and close your lips around it.
- As you begin to breathe in slowly and deeply through your mouth, press down firmly on the canister to release one puff of medication.
- Continue to breathe in slowly for 3-5 seconds.
- Hold your breath for 10 seconds to allow the medication to settle in your airways.
- Wait about 1 minute before shaking the inhaler again and taking the second puff (if prescribed).
- Rinse your mouth with water after use to help prevent oral thrush and hoarseness.
Precautions
• Paradoxical Bronchospasm: Ventolin can sometimes produce life-threatening paradoxical bronchospasm (wheezing and shortness of breath immediately after inhalation). If this occurs, discontinue use immediately and institute alternative therapy. • Cardiovascular Effects: Albuterol can produce significant cardiovascular effects in some patients, such as increased heart rate, palpitations, and elevated blood pressure. Use with caution in patients with cardiovascular disorders (e.g., coronary insufficiency, cardiac arrhythmias, hypertension). • Metabolic Effects: May cause hypokalemia (low potassium levels) and hyperglycemia (high blood sugar). Use with extreme caution in patients with diabetes mellitus and monitor serum potassium in patients at risk for hypokalemia. • Do Not Exceed Recommended Dose: Excessive use of a SABA is potentially fatal. Fatalities have been reported in association with excessive use of inhaled sympathomimetic drugs. • Immediate Hypersensitivity Reactions: Immediate hypersensitivity reactions may occur after administration of albuterol sulfate, as demonstrated by rare cases of urticaria, angioedema, rash, bronchospasm, and oropharyngeal edema. • Coexisting Conditions: Use with caution in patients with convulsive disorders, hyperthyroidism, and diabetes.
Contraindications
Ventolin HFA is contraindicated in patients with a history of hypersensitivity to albuterol sulfate or any other component of the formulation (e.g., HFA propellant).
Possible side effect
Like all medicines, Ventolin HFA can cause side effects, although not everybody gets them.
Common side effects (may affect up to 1 in 10 people): • Headache • Nervousness or tremor • Throat irritation • Muscle cramps • Tachycardia (fast heart rate) • Palpitations
Uncommon side effects (may affect up to 1 in 100 people): • Dizziness • Drowsiness • Irritability • Insomnia • Dry mouth • Nausea • Heartburn • Cough

Drug interaction

Cardioselective beta-blockers could be considered with caution, but they may also antagonize the effects of albuterol. • Diuretics: The ECG changes and/or hypokalemia that may result from the administration of non-potassium-sparing diuretics (such as loop or thiazide diuretics) can be acutely worsened by beta-agonists, especially when the recommended dose of the beta-agonist is exceeded. • Digoxin: Concomitant use can decrease serum digoxin levels, potentially reducing its therapeutic effect. Monitoring of serum digoxin levels is recommended. • Monoamine Oxidase Inhibitors (MAOIs) and Tricyclic Antidepressants (TCAs): May potentiate the cardiovascular effects of albuterol. Albuterol should be administered with extreme caution to patients being treated with these agents.
Missed dose
Ventolin HFA is used on an as-needed basis for symptom relief and is not intended for scheduled dosing. Therefore, the concept of a “missed dose” does not apply. Use it at the next onset of symptoms. Do not double the dose to make up for a missed one.
Overdose
Overdosage with albuterol is likely to produce exaggeration of its pharmacologic effects and adverse reactions. Symptoms of overdose may include: • Angina pectoris (chest pain) • Hypertension or hypotension • Tachycardia with rates up to 200 beats per minute • Nervousness • Headache • Tremor • Dry mouth • Palpitations • Nausea • Dizziness • Fatigue • Malaise • Hypokalemia • Hyperglycemia • Insomnia • Metabolic acidosis • Cardiac arrest (in extreme cases)
Treatment: Discontinuation of Ventolin HFA and institution of appropriate supportive and symptomatic therapy is recommended. The judicious use of a cardioselective beta-receptor blocker may be considered, bearing in mind the risk of inducing bronchospasm in asthmatic patients. Cardiac monitoring is recommended in cases of significant overdose.
Storage
• Store at room temperature between 20°C to 25°C (68°F to 77°F). Excursions permitted between 15°C and 30°C (59°F and 86°F). • Do not puncture or incinerate the canister, even when it is empty. • Do not store near heat or an open flame. Exposure to temperatures above 120°F may cause bursting. • Keep the canister away from eyes and skin. • Keep out of reach of children.
