Common use
Wellbutrin is most commonly prescribed for the treatment of major depressive disorder (MDD) in adults. Its use is grounded in restoring the balance of certain natural neurotransmitters (norepinephrine and dopamine) in the brain. It is also FDA-approved for the prevention of major depressive episodes in patients with a history of seasonal affective disorder (SAD) and as a treatment to aid in smoking cessation. Off-label, it is sometimes used to treat attention-deficit/hyperactivity disorder (ADHD) and to counteract SSRI-induced sexual dysfunction, though these uses are not officially approved and require careful clinical consideration.
Dosage and direction
Dosage must be individualized according to the patient’s clinical response and tolerance. For the initial treatment of MDD using the XL formulation, the recommended starting dose is 150 mg once daily in the morning. Depending on clinical response, the dose may be increased to the target dose of 300 mg once daily after several days. Doses should be administered at least 24 hours apart. The maximum recommended dose for the XL formulation is 450 mg once daily. For the SR formulation, the recommended starting dose is 150 mg once daily, with a increase to 150 mg twice daily (with at least 8 hours between doses) after 3 days. The maximum dose for the SR formulation is 200 mg twice daily. Tablets should be swallowed whole and not crushed, chewed, or divided, as this can lead to rapid dose release and increase the risk of adverse effects, including seizure.
Precautions
Patients should be closely monitored for clinical worsening, suicidality, and unusual changes in behavior, especially during the initial few months of therapy and during dose adjustments. Wellbutrin is associated with a dose-dependent risk of seizure. This risk is increased in patients with a history of seizure, head trauma, brain tumor, or eating disorders (e.g., bulimia or anorexia nervosa), and in patients undergoing abrupt discontinuation of alcohol or sedatives. Use with extreme caution in these populations. May cause elevated blood pressure and hypertension. Periodic blood pressure monitoring is recommended. Can activate mania or hypomania in patients with bipolar disorder. Screening for bipolar disorder is recommended prior to initiation. Use with caution in patients with hepatic or renal impairment, as clearance is reduced; dose reduction may be necessary.
Contraindications
Wellbutrin is contraindicated in patients with a known hypersensitivity to bupropion or any other component of the product. Its use is contraindicated in patients with a current or prior diagnosis of bulimia or anorexia nervosa due to a higher incidence of seizure observed in this population. It is also contraindicated in patients undergoing abrupt discontinuation of alcohol or sedatives (including benzodiazepines). Concomitant use with a monoamine oxidase inhibitor (MAOI) is contraindicated. At least 14 days should elapse between discontinuation of an MAOI and initiation of treatment with Wellbutrin, and vice versa.
Possible side effect
The most commonly observed adverse reactions include dry mouth, nausea, insomnia, headache, dizziness, and constipation. A dose-related risk of seizure is a serious side effect. Other important side effects can include:
- Neuropsychiatric: Agitation, anxiety, tremor, confusion, hostility, and rarely, psychotic episodes.
- Cardiovascular: Tachycardia (fast heart rate), palpitations, and hypertension.
- Dermatological: Rash, pruritus (itching), and in rare cases, severe skin reactions like Stevens-Johnson Syndrome.
- Other: Weight loss, excessive sweating, and tinnitus (ringing in the ears).
Drug interaction
Wellbutrin has significant drug interaction potential. It is metabolized primarily by the CYP2B6 enzyme. Concomitant use with MAOIs is absolutely contraindicated due to the risk of hypertensive crisis. Coadministration with other drugs that lower the seizure threshold (e.g., antipsychotics, antidepressants, systemic corticosteroids, tramadol) may potentiate this risk. Drugs that induce CYP2B6 (e.g., ritonavir, lopinavir, carbamazepine) may decrease bupropion exposure and reduce its efficacy. Conversely, drugs that inhibit CYP2B6 (e.g., ticlopidine, clopidogrel) can increase bupropion exposure and the risk of adverse effects. Wellbutrin itself is an inhibitor of CYP2D6 and can increase plasma levels of drugs metabolized by this enzyme, including many antidepressants (SSRIs, TCAs), antipsychotics (risperidone, haloperidol), and beta-blockers.
Missed dose
If a dose is missed, the patient should take it as soon as they remember. However, if it is close to the time for the next scheduled dose, the missed dose should be skipped, and the regular dosing schedule resumed. Patients should never take a double dose to make up for a missed one, as this significantly increases the risk of seizure and other adverse effects.
Overdose
Overdose with Wellbutrin is a medical emergency and has been associated with significant morbidity and mortality. Symptoms of overdose are often severe and can include seizures, hallucinations, loss of consciousness, sinus tachycardia, and cardiac conduction disturbances (e.g., QRS prolongation). Fatal outcomes have been reported. In case of suspected overdose, emergency medical services must be contacted immediately. Hospitalization is required for symptomatic and supportive care. There is no specific antidote; management involves securing the airway, ensuring oxygenation, and managing seizures and cardiac abnormalities with appropriate pharmacological agents. ECG monitoring is essential.
Storage
Store Wellbutrin tablets at room temperature, between 20°C to 25°C (68°F to 77°F). Excursions are permitted between 15°C and 30°C (59°F and 86°F). The medication must be kept in its original container, tightly closed, and out of reach of children and pets. Protect from light and excessive moisture. Do not store in bathrooms or other damp areas. Safely discard any expired or unused medication.
